I have IBS and I thought it was funny, YMMV. I don't see it as making fun of someone with a medical issue, I see it as making fun of trail culture.
I have only let one person ever pick a name for me. Of course at the time I was in no condition to tell my Mom what to put on the birth certificate
A number of years ago I heard of a guy called Sock in Hole. Don't know if he was real, but it made for a funny story.
This was an 'art imitates life' piece. I have IBS and some friends were suggesting 'Squishy' as my trail name, but fortunately, that didnt happen. I appreciate the kind words, folks. I'll try to come up with another one. I'm planning on a thru hike out in Bankhead Forest next week, so I'm sure there will be some comedic material generated there.
I have met hikers with trail names of ... PrivyBoy and LongDrop.
