Quilt size and shape

I would like to design a quilt and am wondering if there are guidelines on how to fit it to a person's size. For example, should the width be a percentage of one's arm span? Or perhaps it depends on the size and diameter of one's shoulders / torso / belly?

I have a quilt that is slightly too small and lets cold air in through gaps and ridges where it touches the ground. I can increase the size a bit to fix that but am wondering if there are design guidelines.
